What To Complete Before The First Visit

Useful Tips About Insurance

  • Get a prescription form from your child's pediatrician who has the therapy requested, the frequency and duration of treatment, and a diagnosis code.

  • For Speech and Occupational Therapy, print and complete our New Patient Case History Form.

  • For Psychological Services, complete our Psychology Services Background Questionnaire. If you are an adult in need of services, click here.

  • Read our Notice of Privacy Practices.

  • Gather any previous evaluations or treatment information that would be helpful for your therapist.

  • Deliver or fax the information to us at least 2 days prior to your appointment.

Helpful New Patient Information

We are client-oriented and have over 15 years' experience in pediatric therapy.

  • Contact your insurance company to determine if your therapy is covered; if it is, find out your limitations and exclusions.

  • Get all clarifications in writing from your plan administrator.

  • Keep records of all of your correspondence and phone calls, including the date, time, and name of the person you spoke to.

  • If your policy requires a referral, make sure we get it before your first appointment.

  • Notify us immediately if you change insurance companies.

  • Discuss your concerns with your child’s pediatrician and request a referral or script. Referrals should include the service being requested and a diagnosis code.

  • Prior to your child’s first speech-language and/or occupational therapy appointment, please print and complete our New Patient Case History Form.

  • Prior your child’s first psychological evaluation or session, complete our Psychology Services Background Questionnaire. If you are an adult in need of psychological services, click here.

  • Review our Notice of Privacy Practices.

  • Gather any previous evaluations or treatment information that would be helpful to your therapist.
